An application for the issuance or amendment of a helistop approval shall be made to the Director on the prescribed application form and accompanied by—
| (a) | the standard operating procedures referred to in regulation 139.04.3; |
| (b) | written approval from the authority in control of the land; |
| (c) | written approval from relevant ATSU, if a helistop is located inside controlled airspace; |
| (d) | letter of consent from all licensed aerodromes within 2km radius; |
| (e) | particulars of any non-compliance with, or deviations from an appropriate helistop design, operation or equipment standards prescribed in this Part; and |
| (f) | the appropriate fee as prescribed in Part 187. |
