A person who wishes to be appointed as a reservist shall —
| (a) | complete and submit the relevant application form as contemplated in the Annexure to the chief fire officer; |
| (b) | in the opinion of the chief fire officer be of good character; |
| (c) | be at least 16 years of age and not older than 60 years of age; |
| (d) | in the opinion of the chief fire officer, be mentally and physically capable of performing fire service duties in general or any or all of the functions of a service as defined in section 1 of the Act; |
| (e) | obtain the approval of his employer to be appointed as a reservist which approval shall appear from the certificate concerned contained in the Annexure; and |
| (f) | if a minor, produce the written permission of his parent or guardian. |
