| (1) | An approved listed beneficiary who needs housing assistance in terms of the Regulations must complete Form 1 of the Annexure to the Regulations and attach the documents required in that Form. |
| (2) | The completed application form and the attachments thereto must be submitted to the dedicated official in the manner indicated on the application form. |
| (3) | The cut-off date to be determined by the Minister by notice in the Gazette for the submission of applications must at least provide for a period of 24 months after the date of commencement of the Regulations. |
| (4) | The application form must be available at the offices of the dedicated official and on the websites of the Department. |
| (5) | The dedicated official must assist an approved listed beneficiary who requests assistance to apply for housing assistance in completing the application form and ensure that all the required documents are attached. |
